Steps to reproduce:
Watch a flash video through Twitch or YouTube. Right-click on the open window's taskbar button and click the 'Open new window' task.
Result:
The following happens 25% of the time. The new window opens but is completely blank - see attached screenshot. Also, after all the Firefox windows have been exited, the processes (firefox.exe, plugin-container.exe, and FlashPlayerPlugin_11_8_800_94.exe) keep running indefinitely.
Notes:
I am on Windows 8 and v25 of Firefox currently but have also experienced this with Windows 7 and at least as far back as v23. Flash player version at the moment is 11.8.800.94.

Could you test with a new profile, please.

In addition, be sure your GPU drivers are up-to-date.

This still occurs with a new profile.
I have the latest AMD Windows 8.1 GPU drivers (13.150.1.0) for my Radeon HD 7850. I also experienced this before I upgraded to 8.1.
Please let me know if you need anything else.

Yes, I can see this with HWA disabled.

Does it happen if you open this new window from the menu options (or Ctrl+N)?

No it does not, so in summary this problem only shows up when I click 'Open new window' on the taskbar jump list or double-click the Firefox icon on the desktop.
