Steps to reproduce:
Open the website "www.heise.de". The website is displayed wrong, because Firefox could not find the stylesheet "http://www.heise.de/stil/ho/heiseonline2012.css".
Refreshing the site ("F5") does not solve the problem - one has to clear the cache.
This issue happens quite often.
Actual results:
According to "about:cache" the stylesheet should be stored on disk. The path was:
"C:\Users\User\AppData\Local\Mozilla\Firefox\Profiles\rwwvoxky.default\Cache\C\2D\323FAd01".
But neither the file, nor the directories "C" nor "2D" exist.
Expected results:
If the cached file does not exist, Firefox should load the stylesheet again from "www.heise.de"
Coudl you:
1) Clear the cache and try again: https://support.mozilla.org/en-US/kb/how-clear-firefox-cache
2) Try in safe mode: https://support.mozilla.org/en-US/kb/troubleshoot-firefox-issues-using-safe-mode
3) Try with a clean profile: https://support.mozilla.org/en-US/kb/profile-manager-create-and-remove-firefox-profiles

Clearing the cache helps.
But the point is, that this may be difficult for an unexperienced user.
The solution:
If the file does not exist, Firefox should reload that file from the server.

When I started to use Internet (near 2000) and the first versions of Firefox, every user was aware of cleaning the cache because cache size was very small (around ten MB) and often corrupt (with a bad page rendering as result). It was a reflex. ;)
Honestly, having a broken cache is relatively common, and clearing it or forcing caching (ctrl+f5) works in general.

Firefox should work without constantly clearing the cache.

Are you able to reproduce the issue with a clean profile?
If the issue is just random without knowing the reason, I don't think there is something to fix unluckily.
