Steps to reproduce:

1) Login to the Pancare Website as an User.
2) Click on Rewards --> GOHC Service Report.
3) Click on any one record.
4) Click on Deactivate Service.
If I understand correctly your summary and STR this is not a bug in Firefox. 
Did you try this in other browser(Chrome, IE)? If you did and it still doesn't work, please contact Pancare support as the problem is with them.
