When the cookie manager is open and you discover that a cookie is from a site you don't want to accept cookies from, you should be given the chance to block that cookie and permanently block that site from storing cookies at the same time. In addition, there is no way to add sites to the block list except by visiting the site first - add an input box for this as well. A similar idea could be followed for image blocking. That's what the checkbox "don't allow removed cookies to be reaccepted later" is all about. As for the second item mentioned here (it's bad to put two separate items in the same report), that's already covered in other bug reports.
