If you look at the spec you'll see that we have to periodically refresh the ratings from IARC and look for any changes.  This is the GET_RATING_CHANGES API call at the bottom of the IARC docs.  For now, let's say once a day.

Normally just update our rating to be in line with what they send us.  (We should log all ratings changes, but we should be doing that anyway).  The only special case here is if a rating changes from non-adult to adult then we want to put it in the re-review queue with a note saying why.
