Description:
Default call volume is set to max which is quite painful when receiving a call using headphones, especially if the user has intra-aural canal as opposed to intra concha; most headphones (even the new iPhone ones) are canal style.

Repro Steps:
1) Updated Buri 1.3 mozRIL to BuildID: 20131111040200
2) Plug in headphones (preferably canal)
3) Receive a call with max volume.

Actual:
Volume is uncomfortably loud.

Expected:
Call volume is cut in half when headphones are plugged in.

Yes, the ringer volume seems very loud when using headphones in both 1.1 and 1.2.
