Steps to reproduce:
1. Load details page for an app in MP-dev (ex. https://marketplace-dev.allizom.org/app/facebook-1 )
2. Submit a review.
3. Edit your review and press the Submit button.

Expected results:
The review is updated successfully.

Actual results:
"There was a problem updating your review" error is displayed.

Notes/Issues:
Verified on FF28 (Win 7).

Also, the Cancel button from this page does not work, and changes the header into "undefined".
