I'm running on device and the debug button doesn't show up when I click start. The app launches properly on device. If I disconnect and connect to the device the debug button is displayed. Watch attached video
From the video, it seems clear you are using the latest nightly B2G code. What about the Firefox version?

Sorry for taking so long to follow up on this. I see exactly the same problems in the app manager on Firefox 26.0

And I still see can reproduce the problem in current Nightly (29.0a1 2013-12-19)

We landed a bunch of patches yesterday that should fix this issue.
