I will check Survey Gizmo first to determine if we have enough results. If we have enough results, then we can take down the survey on Jan 2. 

If we have enough results, remove stub installer fallback page survey. 

If we have enough results, stop Survey Gizmo and run report of results.

1559 responses for this survey and many useful responses in the empty field. We can remove this survey now.

Opened pull request https://github.com/mozilla/bedrock/pull/1557 just commenting out the survey so we can easily restore/update it when the time comes.
