960830 - Update Brand Engagement Initiation Form

Bug creation: 
Hi. We need to make a few updates to the Brand Engagement Initiation Form and I was hoping you could help.

- Instead of a big text box, we'd like break apart the Creative Specs + Deliverables section into 3 pieces:
 
Header: Creative Specs + Deliverables
1. Select what sort of help you need: copy, design, video, other" (with a text box for clarification) - this just needs to be a simple check list
2. Specs: What is the final deliverable and what format should it be delivered in? Please include information on the format, image/file size, word count, video length, etc. We like details here. - a text box
2. CTAs and design directions: Provide as much information as possible. Make sure to include links to documents with copy, mock-ups, wireframes, or any other information or assets that could help with direction. - another text box

- Switch the creative due date and launch date fields. Also, the due date needs to auto-fill from the creative due date field, not the launch date.

We have a few additional changes that we will want but are still determining what those look like. Please let me know if you have any questions.

Hi David, 

Thanks for making these updates. I have a couple of small changes. 

1. Can you bold "Specs:" and "CTAs and design directions:"?
2. Can you also make those two items plus the Creative Due Date field required? 

I will have one additional update with the goals dropdown, but need to wait until the company goals are final.

This is looking great. 

Two final changes on the page:
- remove the colons behind "Specs" and "CTAs and design directions"
- start a new line with the text after each of the above

As mentioned before the final change will be in the goals dropdown, but I need to wait until the language around the company goals has been shared out.

- Firefox Browser
- Million Mozillians
- Services 
- Org Support
- Other (which continues to bring up the box to type in)
