In many cases, the state on the screen changes, and the screen reader will remain silent and still hold a cursor position to an invalid node. We need to proactively change the cursor position when the state changes. Some examples: 1. When scrolling or swiping in homescreen the cursor should update to nearest visible node. 2. When a dialog pops up the cursor should change to focused or first node in dialog. 3. When dialog goes away cursor should go back to where it was before the dialog came up. 4. When screen reader is enabled the cursor should go to focused item or first item on screen. 5. When unlocking lockscreen cursor should go to active app. 6. When waking display from sleep, there should be speech output as to where the current cursor is.

Closing a 6 year old empty meta bug.. might be more work in this area, but will file accordingly.
