Expected results:

should have got actual login window

FF Version 30.0a1 (2014-02-17)
I am unable to reproduce this
When I go to “www.proteccion.com.co“
Click on the button “Ingresar” under “Afiliados y pensionados” a new whit e box opens and takes a few seconds but populates with the page information for “Accede a tu zona transaccional Afiliados y Pensionados“ which looks correct.

Does the bug happen with the latest Nightly?
Does the bug happen when running in Safe Mode?
Does the bug happen in competing browsers?

Yes to all 3.
I don't know about Windows 7 but I am getting this bug on the mac

This is probably caused by the website itself, since the browser just display what it sends.

I have tested the mentioned issues on latest Firefox release (43.0.4) and could not reproduce it. The web page works properly,  got the same result as @dfren.

Firefox: 43.0.4, Build ID: 20160105164030, User Agent: Mozilla/5.0(Macintosh; Intel Mac OS X 10.11; rv:43.0) Gecko/20100101 Firefox/ 43.0

Can you please test this on the latest Firefox release (43.0.4) or latest Nightly (46.0a1, https://nightly.mozilla.org/) and tell me if this still reproduces for you ? When doing this please use a new fresh Firefox profile (https://support.mozilla.org/en-US/kb/troubleshoot-and-diagnose-firefox-problems).
