Is this case-insensitive by name? Or by URL? Please clarify.

By name, as they appear in the list. It's because many web sites start their web
page title with lower case and so those are all at the end of the list, which is
pretty annoying. For example: java.sun.com, jxta.org, heise.de, webwasher.com.
