When we create an account with an smtp server it should always use the
associated smtp server.

This has been tested by QA on a test build and certified as a safe fix -
nominating for branch.
The bug for the duplicate smtp server is the following one.
http://bugzilla.mozilla.org/show_bug.cgi?id=132948 - Add a second IMAP/POP
account & a duplicate SMTP server appears in the SMTP list.

Verified/Fixed on the branch after trying the following scenarios:

Created a variety of multiple accounts in one profile and they were all  
successful in send/receiving messages.

profile) - is the problem fixed on an old profile or should I trash my prefs.js?
