Turn on external programs several protocols by default

This patch turns on external applications to take care of several protocols
referenced in related bugs, hopefully clearing up those bugs. For some of the
protocols referenced in this bug I'm unsure if I have chosen the "best"
application, so suggest better apps if you wish.

Did fast cleanup for RC2. Still hoping to do fuller re-write for 1.0.

I don't know what that does, inline, but it does display file:///$APPDIR in URL bar.

I rewrote the top section so the URL schemes are rationally organized.

reminder: remove references to 1.0 branch after 1.4 (because most products will
jump branches, and not care about behavioral changes vs. old branch).

I don't think that this is still needed
