When I click on outgoing server and then advanced tab I see all my choices for
smtp servers and I can add them and select different defaults.
If I click on a specific email account and then on advanced button the only
thing I see is several copies of the default server and all the other entries
are blank. Unable to select anything other than the default in this box.
I have tried deleting all the entries and rebuilding them, and deleting entries
from the various preference files also with no success. This but has existed
since the first version of mozilla.
Most email accounts require you to use their own smtp server now. So smtp server
should be one of the settings asked for when you create a new email account.
Otherwise the multiple email account feature is not very usable.
Reproducible: Always

would appreciate advice on how to clean out the prefence files manually so I can
rebuild the smtp servers, if possible.

Are you using the @ symbol in any of your usernames for imap/smtp logons?

The problem exists across platforms (I tested Linux, FreeBSD, Win2k, MacOS9, and
OSX) so the owner or such may want to change the OS to All. As suggested it
does seem to happen every time an "@" is used in a login name for an SMTP
server. It's really annoying ;)
Yes I am using the @ symbol! :)
I am on three different ISPs that use the Imail server by Ipswitch and they all
require that you enter the entire email address as the username.

I am also seeing this problem on Linux with a 200301xx build (I'm not at home
right now, so I don't recall the complete build number although it is within the
past week). I have two outgoing smtp servers defined which can be seen in the
Outgoing Smtp->Advanced dialog. The non-default server requires the login name
to be 'LoginName%Domain'. When I then go to set the smtp server for an account,
the dialog shows 'Use Default Server', the other smtp server I have defined, and
a blank line. If the blank line is chosen (as that's where the server should
be), after clicking Ok and re-opening the dialog 'Use Default Server' is checked.
