200908 - cannot add URL to bookmarks. 'OK' button not responsive

Adding URL to a bookmarks folder is not functioning. The Add bookmark appears,
but the 'OK' button is not responding, hence the URL is not added. The Add
Bookmark screen should be removed with the cancel button.

Reproducible: Always

Steps to Reproduce:
1.Bookmarks
2.Add to Bookmarks...
3.OK

Actual Results:  
Nothing whatsoever. The Add Bookmark screen does not dissapear and the bookmark
is not being added

Expected Results:  
Add bookmark screen should dissapear and bookmark should have been added to the
folder specified

Used with the default theme
