The ATK accessible hierarchy has changed unexpectedly on 9/17 or 9/18.  This may indicate some sort of problem.   It looks like the missing items have parents of role unknown whereas before they were direct descendants of the frame.
I'm confused: if they are missing then how can you tell what their parent is?

It was speculation from what I have seen in the past, however, it really makes no sense now that I read it out load.  It seems that most of the missing accessibles are menus and two tooltips.  I wish I had more for you to go on.
I'm not sure if this is another case of the same bug, or a different bug, so I'm starting here. :-)

If you examine the contents of the multi-column tree of bookmarks in the Places Organizer, you will see that each bookmark has role unknown.  In addition, if you launch Orca and then arrow among these bookmarks, the following error is generated for each row that gains focus:
