image dialog--constrain is not hooked up for image sizes

in the image dialog, the constrain checkbox is ignored (for keeping the width and
height in proportion with each other)

This is working now except that it doesn't account for pixels / percent. Maybe
constrain should be disabled if one of the values is %? What do you think?

4.x behavior on Mac is to disable "constrain" if one or both popups are set to
percent. I think we should go with that behavior.

You should be able to verify this today since I've checked in the fixedup image
dialog.
