This issue has been plaguing me for a long time (even prior to 2.26).
I have no concrete idea what causes it so I don't know the STR.
I think it might have something to do with SM becoming black momentarily
(probably due to near OOM conditions).
I'm attaching a pic of what it looks like. (Sanitized first, :) )
