This is reproducible on Linux, in the "webide" tool. (Tools > Web Developer > WebIDE) (devtools.webide.enable needs to be true). The top left panel button has an icon (a project needs to be imported first).
This icon is 128x128px, downscaled to 18x18px. It's first rendered pixelated, then after resizing the window, it's rendered smoothly.
