The FxOS device clock sometimes sets to a wrong time. It's hard to provide STR, but sometimes the alarm clock goes off a few hours before it should. Sometimes the clock sets to weird times and rebooting does not help. It just sets to a different (incorrect time). My workaround so far was: 1) Disable automatic time 2) reboot 3) Re-enable automatic time. This bug has been widely reported on dev-b2g: https://groups.google.com/forum/#!topic/mozilla.dev.b2g/dwKfhOINuRQ
I filed bug 1032101 just before this on the flame devices producing the wrong time, with some possible STR there.
This was indeed observed on a Flame, with your steps being more comprehensive. Marking as duplicate.

I have experienced the alarm clock going off at total random times long time ago with Peak 1.2. It started out ouf nowhere (no update) and after a few days of this happening I stopped using the phone altogether. But I never realized the clock switching to the wrong time. So these issues may not be coupled. The other bug is only about the clock issue though.
