Engineering Refactor: Streamline sendMMI(), cancelMMI() and dial() 

AC: All existing user stories from past releases should be validated after this refactor user story completes.
+++ This bug was initially created as a clone of Bug 889737 +++ Bug 889737 is a huge patch, so we are going to split it into smaller pieces in order for the attempt on moving each part (development, review, QA testing, ect) forward in parallel. The meta bug covers the following pieces: 1) Bug 889737 - [MMI] Unify both sendMMI() and dial() functions 2) Bug 1058397 - [B2G][Telephony] add UssdSession API
