STR : 
- Open the toolbox
- Go to http://smartsearch.altervista.org
- Wait for the page to load
- Click the switch frames button

See that there are 3 about:blank frames. The 2 first about:blank frames can't be switched to. The third about:blank frame can be switched to and corresponds to the #results-frame-extra.

Note that this problem doesn't happen if you open the toolbox *after* the page has loaded.

@Alex, these about:blank frames are created by gecko multiple times while creating any global. Are you not blacklisting them ?
