Proof of concept more or less done.

Form appearance needs some work to make it more readable.

We might want to connect more of the fields to semantic properties so they are browseable/searchable.

(Note: all this work is on dev so don't put any real data you want to keep there)

I've re-created this form and associated items on the production wiki.

The text will need to be finalized on all these pages after review by stakeholders (lead by Kirstin). 

I suggest we add some fields to aid in sorting and browsing of submissions. Ideas include:

* target region
* submission status (e.g. submitted, approved, in-progress, complete, etc.)
* short summary (for use on listing pages, social media, etc.)
* budget

I also suggest we consider changing 'impact goal' from a free-form field to set list that corresponds to our goals.
