1071578 - [Dialer] Incorrectly receive Missed call notification for a conf call participant

Steps to Reproduce:
A = Flame user. B, C, D are just other phones
1.  A Calls B.  B answers
2.  C calls A.  A answers (B is now on hold).
3.  A merges B & C into a conference call.
4.  D calls A.  A answers and conf call put on hold.
5.  B Hangs up.
6.  A receives a missed call notification for B.

I am also observing (intermittently) that C is sometimes dropped when B hangs up.
