Hi, 

We would like to create a simple form to take in requests for the Firefox costume. Thank you for your help! And please let me know if you have any questions or something doesn't make sense.

Looking to get a Firefox Costume for your upcoming event(s)? Please use this form to tell us about your event(s). Filling out this form does not guarantee that you will receive the Fox costume. Requests are reviewed weekly. We will get back to you with next steps after we have reviewed. 

Before submitting your request, please make sure to review the Firefox Costume wiki pages [link to https://wiki.mozilla.org/FirefoxCostume]. You’ll find all the information you ever imagined needing, including details about what we’re looking for in your request, how it’s decided where a costume will be sent, and details about the actual costumes.

Hi Jennifer, we're just getting back from #mozlandia and finishing up some Q4 goals.  Is this form for a particular project with a deadline?  It looks fairly straightforward, but there's not much time left in the quarter.  If your form isn't critical, it would be easier for us to start on it in January.

Jennifer and I talked about this and we're ok to get this done in January. The earlier the better in January but definitely don't need it right now. 

We can circle back early January to see when we get this finalized.

what product/component should this form create bugs into?

Product should be marketing. 

I'm not so sure about component. Is it better to use an existing component or to create a new one? If using something that is existing is better, either general or community would probably work. Although, I'm not sure how either of these is currently used. If something new could be created, maybe something like event assets or costume requests?

for high volume forms i'd suggest a new component.  however i doubt you'll be flooded with costume requests, so let's go with "general".

Thanks for the reminder. This looks good. One hopefully small change - is it possible to add one section at the end. The header should be "More Information" and then a text box with "Additional Notes, Comments, Etc."
