Steps to reproduce:

On the weather channel website there are flash videos. If I click to watch a video I see a gray screen. There is a banner at the top asking me to activate flash. If I click on the gray screen it asks me to allow flash. Firefox does not show the 'Activate Adobe Flash' graphic in the weather channel's gray box.

Tested Firefox 31 ESR, 36.0a2, 37.0a1

Expected results:

When flash is 'ask to activate' Firefox should have the 'Activate Adobe Flash' with the lego icon in the video area so users know they will have to activate it to watch the video. It is currently like that for most sites but not for the weather channel for some reason.

Can you try with a fresh new profile and see if you get the same behavior?

Normally if you go to about:addons -> plugins you will see that Shockwave Flash is Always Activate. So you would not need to activate the plugin. Also check if your plugin is up to date.

I have flash set 'Ask to Activate'. Yes, I can reproduce with a clean profile.

I have bad feeling having small flash apps in my design...

If the Flash <object> tag too small, i only see ugly grey rectangles,
without any graphics or "Activate" messages.

Resolving old bugs which are likely not relevant any more, since NPAPI plugins are deprecated.
