I don't know what the exact trigger for this is, so I'll describe what happened the last time this showed up. I have a 27" dell non-hidpi display and a 15" Early 2013 Macbook pro w/retina screen and Nvidia graphics. I usually keep the lid closed (so only one monitor) when I use my external monitor. This means that all my windows are switching between being on a Hidpi display and a normal one without being explicityly moved using the trackpad. Just now, I accidentally bumped the DP cable. This caused the screen to switch from normal to hidpi on the laptop panel, then back to hidpi. Reloading a page is enough to fix this. Tabs which were in session restore's lazy-load limbo state load properly when I select them. All loaded pages end up in this top left state, even if they are not focused. I'm running the latest nightly with E10S turned off.
I think this is a dupe of bug 1125325.
