I've noticed over the past few days that intermittently I will get multiple tabs for a page I only open using the homescreen bookmark. I believe nightly recently added the behavior where these bookmarks should always open the same tab.
Unfortunately I don't have clear STR yet. This morning I saw it after these steps, but I haven't been able to retry them:
1) Have nightly running with HS bookmark open in a tab.
2) Nightly auto-updates killing the process.
3) Open the Android task switcher and select Nightly. I believe this re-launched the new updated executable and reloads saved state.
4) See that the previously opened HS bookmark tab reloads.
5) Click on the HS bookmark again.
6) Open nightly tab switcher and see that there are now two tabs for the HS bookmark.
In my case the bookmark was twitter.
Nexus 5 Android L. Nightly 38.0a1 2015-02-05.

Perhaps the URL for the open tab changed from the original homescreen bookmark URL?
I suspect we just compare the URLs directly, and may not account for things like the hash changing.

The site was m.twitter.com. as far as I can tell, the URL for the two tabs was identical. I don't think twitter normally uses hash fragments.

Thanks for the info. I think mfinkle has ideas about what might be going wrong here. It's possible that we're not restoring these "pinned" tabs correctly if your browser is killed in the background.
