Maybe, this seems to only happen after several crash(bug 1133306) and restart browser. Too many firefox.exe process remains in Task Manager.

This can be caused by the audio mixer process being confused by all the crashes and not accepting all connections.

Tracking 37+ as the description in comment 0 reads like a regular enough use case. Paul - Comment 2 makes it sound like you have an idea of where this bug may come from. Can you take this one to investigate?

bug 1134258 and bug 1134263 are going to make this happen way less. In _extreme_ cases, and depending on the hardware, this will still happen, though, and there is not a lot of thing we will be able to do.
