1154226 - Compile list of FHR v2 and v4 data fields and their mappings for validations

We were working from FHR v2 data field lists before when compiling the v4 tasks, but now we need a clear list to work on.

We should:
* collect data in a shared gdoc spreadsheet
* list v2 data
* list the corresponding v4 data next to it (or point to the pending bug)

We will probably extend this later with the v4-only fields for proper QAing and to kick off more complete documentation.
