I see only one square. I don't see the top one.

This is a driver bug to the best path forward is to update the driver. Alternatively you can disable acceleration from about:preferences#advanced by unchecking "Use hardware acceleration when available" but this will make things go slower. We are working on a mechanism to automatically detect this kind of bug.

I can see the top square at http://people.mozilla.org/~ajones/blank-video/ when disabling hardware acceleration, but I still can't see the video at http://www.playboy.com/videos/handjob-karaoke-japan (but I can see it on another computer with Linux). (Note that I have to manually disable HW acceleration, probably because the fix hasn't been released yet?)
I can still reproduce this issue.

I've just tested it on Nightly, and I can't see the video on Nightly as well. about:support on Nightly says: > Supports Hardware H264 Decoding false

The plan is to reproduce the test you're doing using js in bug 1156135.

While I'm tracking this for 39, it's marked fixed for 39. So I only came across it by chance. Is this problem still showing up?

It's fixed in Firefox 41. I can see the two squares on http://people.mozilla.org/~ajones/blank-video/. The video on http://www.playboy.com/videos/handjob-karaoke-japan is using flash now, so I can't test it. The flags probably need to be updated to reflect that this is broken on Firefox 39 (and probably Firefox 40).
