somewhere during work since it initially went live, bug-modal stopped working in chrome.  there's a few minor layout issues, but 'save changes' doesn't work at all right now (no errors, save is disabled then nothing).
the inability to save changes turned out to be a old webkit bug - https://bugs.webkit.org/show_bug.cgi?id=14443
i've worked around it by explicitly calling form.submit() in our click handler.
