Details

Sometimes the URL-bar will not take focus, meaning that you can't enter
anything. Only resolution is to close browser instance.

To reproduce:

Quicklaunch enabled:
Open browser using shortcut to mozilla.exe
Minimize browser
Open new browser using shortcut to mozilla.exe

The new browser instance is born without focus and nothing can be typed into the
URL bar.

I've seen this behaviour on different windows 2000 and XP systems.
