Overview: If a card doesn't have a generated name then it does not have a title
when printed.

Steps to reproduce:
1. Create a card which only has an address (i.e. as in the Collected AB)
2. Select the address book
3. Print the address book (File|Print Address Book)

Actual Results: It prints the card but it's not fomatted nicely. It begins
with the email address in Normal text and it's difficult to tell where one card
ends and another begins because all the text appears normal.

Expected Results: In this case use the email address as the title. After
printing the address book the email address will appear first in bold. This will
help differentiate this card from the previous card.
marking nsbeta1 so that printing an address book is easily readable if a card
only has an email address.
