disable bug-modal then file a bug.  the bug is shown using bug-modal.  refreshing reverts back to the standard ui.
this only happens when a format param is passed to enter_bug.

the core issue is enter_bug's format param was being passed to show_bug, however they serve different purposes.  i've fixed that, and shored up the modal extension's handling of unexpected formats.
