So far, this has occurred once in firefox-media-tests. 

Summary of this test run: 6 videos started playback successfully (played first few seconds of each video), then Flash was requested for video KlyXNRrsk4A, then all the videos played entirely (including KlyXNRrsk4A).

I wonder if this is related to bug 1202100, where someone reported YouTube switching from HTML5 video to Flash while the video is playing.
Can we set up telemetry on MediaSource errors? This may be triggering fallback.

I could easily reproduce it when mediasource webm was enabled.

There are still a few decoding error occurring and when that happens almost instantly; YouTube switches back to flash.
Caught this in automation again: still in 42, and again the same YouTube URL. See https://bugzilla.mozilla.org/show_bug.cgi?id=1182513#c32.

The failing video often includes an unskippable ad at the beginning, which might be why it is able to trigger this failure.
