STR:
1) Visit https://www.youtube.com/watch?v=eQ0DEFaSonE, and once the ads are gone, watch the first few seconds of Mostly Walking: Loom Episode 1
2) Open up a different browser (I used Safari), and browse to the same video and watch the same first few seconds.
ER:
The audio and video should be played back in sync.
AR:
In Firefox, the audio seems to be a few seconds ahead. Seems fine in Safari.
I'm on OS X on Nightly (47.0a1 - 20160218030349).

What happens if you set media.mediasource.webm.audio.enabled to false?
If that fixes it, it's YouTube encoding issue again.

Yep, that fixes it! By "YouTube encoding issue", does that mean that the problem is on YouTube's side of things?

Yes. It is a YouTube encoding issue. YouTube said those were very rare and said they will take action to re-encode them automatically. Looks like they haven't done it yet.
See bug 1245696.
