There's a bug on WebKit with a reproducible test case: https://bugs.webkit.org/show_bug.cgi?id=149953
I just tested in Safari 9.1 and can confirm that the bug still exists.

Thanks for the poke — the info was actually correct on the IndexedDB ref pages, but wrong here. I've fixed it now.
