I had this same behvaior. Installed a fresh copy of Firefox 45.0.1 x64 bit, and it does not install the openh264 and primetime plugins. It didn't not work on a fresh profile nor on a complete reinstall of firefox. It has worked correctly previously on some earlier version of Firefox.
Our AUS server that services our OpenH264 plugins is offline. It should be back up soon, but until it is, a user will need to manually fetch the plugin and install it in the correct directory if they want to use OpenH264. I'll keep this bug open until the AUS server is back up. I expect we'll get several dup reports about this, and we can use this bug to dup them to.

I'm trying to help a user with this issue over at https://support.mozilla.org/es/questions/1116447 How does one "manually fetch the plugin and install it in the correct directory" exactly? ¡Gracias!

¡Hola Alex! The AUS update server is back up; I just verified it's installing OpenH264 plugins as it should. The user may need to restart their browser to force a check. (Checks are made once per day in a running browser.)
