Hi, 

The Legal team would like to do something similar to the Marketing team, which displays custom bugzilla forms at the top in a yellow box, when a new Marketing bug is opened[1]

I have a few custom forms. The first is for NDAs (which already exists as a component in the Legal bug).  The NDA questions are below. Required questions are designated with *


NDA Questions:
(1)* Do we really need to receive confidential information from the other party or provide them with ours at this stage of the relationship? Y/N
(If yes, please explain - open field)

(2)* What is the purpose of exchanging confidential information with the other party?

(3)* What confidential information will Mozilla be sharing?

(4)* What confidential information will the other party be sharing?

(5)* Other party info:
*Party Name
*Business Address 
*Name for contact person
*email address for contact person
