Steps to reproduce:
Edited PDF and uploaded edited PDF to WordPress site. Clicked on link to new PDF in Firefox (on the Mac).
Actual results:
Firefox displayed old (unedited) version of PDF. Clicking on the download icon will download the correct (edited) version of the PDF. The downloaded version displays correctly in Adobe Acrobat. Also, clicking on the link in Safari and Chrome displays the correct (edited) PDF. This is confusing to the website visitor, since the old form contains an old 2009 date in the headline.
Expected results:
The latest version of the PDF should have been displayed in Firefox. Very confusing, since the downloaded file and the file viewed in Firefox don't match.
!!! In fact, simply opening the attached file in Firefox shows the incorrect view of the PDF. The Headline reads "Medical Form for 2009 Imprint Tours" instead of the correct "Medical Form for Imprint Tours" without the 2009. The attached file views correctly in Adobe Acrobat as well as MacOS Preview.app.

I have tested your issue on Windows 8.1 x64 and Mac OS X 10.11.6 with the latest Firefox release (v48.0) and latest Nightly build (Build ID: 20160808030441) and managed to reproduce it.
I have opened in a new tab the attached PDF file and observe that the title was "Medical Form for 2009 Imprint Tours". The next step was to download the attached PDF file on the local machine and opened using a PDF viewer (Foxit Reader for Win and Preview for Mac), the title was "Medical Form for Imprint Tours".

It seems pdf.js has issues parsing the pdf and has to go into recovery mode:
