Tested with LG G4 (Android 6.0.1), HTC Desire 820 (6.0.1) and Asus ZenPad 8 (Android 6.0.1) and couldn't reproduce what you described. After I enter the URL, a door-hanger was displayed asking to receive notification from serviceworke.rs. Tapping "always" and "request sending a notification" for 5 times, got me 5 notifications in android notification bar. 

What device were you using? You have any add-on installed?

I'm using a Sony tablet with Android 6.0.1. If it is only me having this problem, then you should focus on something else!
 
Anyway, when entering that URL on Firefox stable or beta I get the doorhanger as you describe. When entering that URL for Nightly I do not get any doorhanger (and no push notifications).

Strangely, for stable, beta and nightly, if I look into settings/notifications, or click the padlock and then select "Edit Site Settings" there is never a notification setting recorded for this site (and this is true also for sites when you select "always" in the doorhanger). Also, if you check the box Allow Notifications in "Edit Site Settings" and then open it again it is unchecked (for stable, beta and nightly).

I have no add-on installed.

I have solved this problem now by uninstalling and re-installing Nightly. I think it would be appropriate to just close this bug now.
Based on comment 3 I will close this as worksforme. If this is still reproducing feel free to reopen it.
