Details

For optional permissions, we need to present a dialog to the user when an extension requests a permission.  Information available at that time includes details about the extension (ie name, icon, etc) and the list of permissions being requested.
This bug is to create the visual design for that dialog, including the copy, where it should appear, etc.
The dialog itself should follow the same design that we use for the other permission dialog, with different copy in the title - mentioning that those are permissions additional to the permissions the extension has been granted on install.
The big question here is where to anchor this dialog as we do not necessarily have a tab related to the extension open when this dialog is triggered. And we do not have a generic modal dialog ready either to use (like chrome uses). Therefor the best compromise would be to anchor the dialog to the extensions toolbar-button if it has one, if not, anchor it from the Firefox menu on the right side of the browser.
