Steps to reproduce:

1. Go to the top right 3-stripes menu
2. Click Customize
3. Move the report Site issues to the toolbar

Report Site Issue button disappears. No icon is shown in the toolbar. To bring it back you need to restore defaults and loose all the other configs.
