I'm able to reproduce a ghost window fairly reliably on www.tvn24.pl. I go to the page, start playing a video, click around a little, close the tab, maybe while the video is still loading.

The window is being held alive by a missing reference to a Promise:

I'm having some trouble reliable reproducing this issue, which will make it difficult to investigate.

Bug 1344357 involves seeking video and has a good test case, so I'll wait until that is fixed to look into this more.
