I'm able to reproduce a ghost window fairly reliably on www.tvn24.pl. I go to the page, start playing a video, click around a little, close the tab, maybe while the video is still loading.

Originally reported in bug 1326095 comment 45.
The window is being held alive by a missing reference to a Promise:

The window also does not show up in a concise log, so maybe there is some other issue, too.
I'm having some trouble reliable reproducing this issue, which will make it difficult to investigate.

Bug 1344357 involves seeking video and has a good test case, so I'll wait until that is fixed to look into this more.
