AFAICT the WorkerFetchResolver's PromiseProxy should be keeping the Worker alive via a Holder. I'm not sure how the WorkerPrivate mutex could be bad here. Andrea, any thoughts?

This intermittent failure has been fixed a couple of years ago. It's hard to say what bug fixed it because the code has change a lot since then.
