Form autofill stores US phone numbers in the form [phone]. I never want to enter phone numbers in that form (without punctuation) as it's ugly and hard to read. Some web sites force that format, but most allow nicer strings to be entered.

Steps to reproduce:
1) Autofill saved information including a phone number.

Expected result:
2) Praise the feature and submit the form.

Actual result:
2) Curse the feature.
3) Delete the ugly auto-filled phone number.
4) Try to select a reasonably formatted number from the field history. (Works about 90% of the time.)
5) If step 4 fails, enter the phone number by hand with reasonable fomatting.
6) Submit the form.

Solution:
* Either save the format from the user, or allow the user to specify a default format to use.
