nexus-5-01 attached to autophone-1 has been unstable which led me to disabling it in bug 1410413 Van, can you check on its connection and/or change its usb cable and/or move it to a different port on autophone-1?

Van moved the device from the chained usb connection on the cooler to a direct port. It seems ok and I'll re-enable it.
